HAZLETT, Thomas Henry, (Chook)

On Sunday, November 14, 2021 peacefully at home, surrounded by his family and friends; aged 60 years young.

Dearly loved son of Kathleen and the late Sam Hazlett, loved former partner of Megan, much loved Dad of Hayley Hazlett, Henry Hazlett, Manu Gibbs, and Chris Gibbs, loved Grandad of of all his grandchildren. Loved brother and brother-in-law of Janette and Tom Sharp, Sandra Hazlett and the late Mac McLaughlan, Sam and Lesley Hazlett, Fiona and Michael Guildford, loved uncle and great uncle of all his nieces and nephews. Will be sadly missed by his family and friends.

Special thanks to all the staff of the Otago Community Hospice and to Sandra Knopp and Dr Janine Winters for their care and support of Chook and his family.

As per Chook’s wishes, a private farewell for family and friends will be held.
